The First 4 Days of the Pre-op Bariatric Diet

The first 48 hours on a liquid diet is the most challenging. You will be very hungry until your body induces ketosis. Ketosis occurs when the amount of carbohydrate fuel (fuel that is needed to run the body) drops below a critical level, forcing the body to turn first to protein and then fat reserves to do the work carbohydrates normally do. You must drink 64-96 ounces of water daily to remove the waste products in the blood stream.

My diet has begun in earnest and I’m four days in. I won’t lie – it’s not easy. But that’s what we tell our patients. You need to work hard for your longer-term success. The first few days are the hardest as your body weans itself off the high fat high sugar foods. The result is cravings that mimic withdrawal from a drug. Below is my menu for the first four days of the diet.

Menu Day 1

  • Breakfast – protein shake
  • Lunch – it was Dr. Feteiha’s Birthday, so I indulged a little more. Salad, chicken, pasta with cheese and a lemon tart…
  • Dinner – salad – lettuce, cucumber, celery, red pepper and olives
  • Water – 64 oz

Menu Day 2

  • Breakfast – protein shake
  • Lunch – salad – lettuce, cucumber and celery with low-fat dressing
  • Snack – 40 calorie ice pop
  • Dinner – soup – low fat cream of chicken
  • 8pm – protein shake
  • Water – 64 oz

Menu Day 3

  • Breakfast – Bariatric Advantage shake
  • Lunch – salad – lettuce, cucumber and celery with no-fat dressing
  • Snack – 40 calorie ice pop
  • Dinner – soup – low fat cream of chicken and a small green salad with low fat dressing
  • 9pm – protein shake
  • Water – 64 oz

Menu Day 4

  • Breakfast – Bariatric Advantage shake
  • Lunch – soup – low fat cream of chicken
  • 3pm – protein shake
  • Snack – 40 calorie ice pop
  • Dinner – salad – lettuce, cucumber and celery
  • 8:30pm – protein shake
  • Water – 64 oz

How I feel

There was definitely a learning curve, going from regular eating habits to the two-week pre-op diet. Day one was a lot easier than day two. But days 3 and 4 were the hardest.

Challenges

It’s hard to ignore celebrations – we had a couple birthday parties and it’s really easy to cheat. On day two, I started developing a headache because I just hadn’t had enough liquid. You come to realize how much of the liquid you consume every day comes from food and not necessarily drinks. Addressing hunger without overeating was definitely a challenge.
